OCUL Outstanding Contribution Nomination Form

Please complete this online form to submit a nomination for the OCUL Outstanding Contribution Award.

A nomination must include:

A stated timeframe for the nominee's contribution (must be between May 2025 and May 2026).
A description of the nominee's contribution.
A letter of support for the nominee.

Review Nomination Guidelines to ensure your nomination is eligible.

Nominations are accepted until Friday, October 2, 2026.
